Death of the Retiree and/or Surviving spouse, what do family members need to do?

In the event of death of the retiree and/or surviving spouse, the family members need to complete the following:

• Notify the Retirement Plan for the CTA employees at 312.441.9694 of the retiree's death and file start the claim process
• Submit an original Death Certificate to the Plan
